Students going into Kindergarten or 6th grade MUST provide state-required medical information to complete the registration process.
All students entering kindergarten and 6th grade are required to have a current physical examination completed on the approved Illinois Department of Public Health Form.
A sports physical does not meet the requirement, except 6th grade physical exam will be acceptable as Sports physicals. 6th grade only.
A current physical is one that was completed within one year and must be completed by September of 2022.
The parent portion on the upper backside of the form must be completed and signed.
All students must be up-to-date with the State of Illinois immunization requirements.
Students will not be allowed to start school unless all mandated health requirements are submitted.
